Terms of Service
In this document: AXTUALISED refers to the owner, administration staff affiliates, or contractual partners and hereto referred to as “we” “our” “us”.
This notice lays out terms of service that you agree to if you should receive services or enter into an agreement with AXTUALISED. AXTUALISED provides coaching, consulting, and clinical therapeutic services to individuals 18 years or older. AXTUALISED does not provide crisis services. If you are experiencing a medical emergency, please dial 911. If you are in an emotional crisis and need immediate mental health attention, call 988 or go to your nearest emergency room.
We aim to provide bespoke professional services that optimize your personal and business growth. Your success depends primarily on your motivation, commitment, expectations and follow-through. We cannot and do not guarantee that you will attain a particular result or outcome, and you accept and understand that results differ by everyone. Please note if you are a coaching client or we have entered into a consulting agreement, there may be different terms of service, and we can discuss further.
Website Usage: Please use our website for lawful purposes. The content crafted on the website is original work so do not copy, distribute content, text, or images without permission.
Technology and Data: Please see our privacy practices and notices. We do use HIPPA compliant software and phone services. However, there may be a time when other non-HIPPA complaint platforms or communication methods may be used.
Service Time: Service times may vary due to the various services we provide. In general, we perform private pay services in 30-minute increments for private pay clients. For individuals who utilize their insurance, sessions are up to 90 minutes for an intake and 45 minutes for follow-up sessions.
Insurance Clients: AXTUALISED may accept insurance as a form of payment for services. As the client you are responsible for co-pays, late, and no-show fees. While most insurance policies cover psychological services to some extent, coverage can vary. It is your responsibility to check carefully to determine the exact nature of your coverage. AXTUALISED will check your insurance eligibility. If your insurance is not accepted or you do not have coverage, you will be responsible as a self-payer.
Co-pays are expected at the time of your appointment. If seen virtually, you will be asked to pay prior to the session starting in an agreed upon method. If your insurance has a deductible you are required to meet, the payment is expected at the time of your visit, even if virtually. If at any time financial difficulties present a problem in keeping your account current, please discuss the issue with AXTUALISED as soon as possible. Failure to make timely payments for services may result in an inability to continue services. If you cancel less than 24 hours or no show, you will be charged a $50 cancellation fee unless we discussed an alternative arrangement. AXTUALISED may revoke their right not to accept insurance or payments; you will be notified of this change and if you choose to stay with AXTUALISED you are responsible for rates.
Private Pay Clients: AXTUALISED accepts cash, credit cards via VENMO or PayPal; additional payment methods may be accepted. You are expected to pay at booking. If I need to cancel or reschedule a session you will not be penalized, and you will be entitled to a refund if we did not meet. If you cancel more than 24 hours before the session you will be able to reschedule within 2 weeks or when the schedule allows. If you cancel less than 24 hours or do not show, you will be charged the full session, and no refunds will be given. If you attend our appointments late, you may not be given the full session time. If there is a reasonable circumstance for your tardiness or cancellation, it may be excused but please contact AXTUALISED.
Emergency Coverage
Please note that AXTUALISED does provide any emergency or crisis interventions. You may leave messages for AXTUALISED during office hours 401-210-9844. If there is an emergency, follow the instruction on the AXTUALISED voice mail message, directing you to call 988, or to call 911 to be transported to your local emergency department. There are no on-call services so please ensure you follow these instructions.
Benefits and Unintended Emotional Risks:
Most people who obtain behavioral health and coaching services benefit from the process. However, during sessions some people may experience uncomfortable feelings such as sadness, anger, guilt, or frustration. Sessions may involve discussion of unpleasant events and situations in your life and may cause unintended emotional distress temporarily. Many people experience a reduction in discomfort over time, as well as improved relationships and better problem-solving abilities, and overall well-being. However, there are NO guarantees about what your specific experience will be. You are encouraged to ask any questions you may have about interventions throughout your sessions.
Dual Relationship:
AXTUALISED will not engage in a dual relationship; meaning, AXTUALISED will only provide one service to a client. A current or former client of behavioral health services, AXTUALISED will not be provided coaching or consulting services together or in the future. Likewise, if a current or former coaching client is currently or has formerly received consultation from AXTUALISED; AXTUALISED will not be able provide behavioral health services in the future. During the work, if AXTUALISED believes that a client needs behavioral health intervention, the client will be referred to a therapist. The client has a right to decline referral. If a client attempts to cross boundaries by trying to engage in a dual relationship or if AXTUALISED cannot treat a client safely, AXTUALISED will refer the client to another therapist and the client will be discharged from AXTUALISED.
Communication Policy to Include Email and Text:
AXTUALISED does not prefer to use email or texting; phone calls are the preferred method. However, we may use email to transmit reports or to communicate for appointment management as appropriate. Please do not email me detailed content related to your treatment sessions as it is not completely secure or confidential. If you choose to communicate with us by email, please be aware that all emails are retained in the logs of your and AXTUALISED Internet service providers. While it is unlikely that someone will be looking at these logs, they are, in theory, available to be read by the system administrator(s) of the internet service provider. You should also know that any emails AXTUALISED receives from you and any responses that we send to you may become a part of your legal record. AXTUALISED will read and return emails during regular business hours only as discussed with you by AXTUALISED. Texting may be used only for appointment management. No personal or clinical information should be exchanged via text. Texting may not be checked regularly so it shall not be used for urgent matters. Please note that although we use a HIPPA complaint texting service, nothing is 100% secure. Texting fees may apply; it depends on your cellular service and the client is responsible for applicable fees. AXTUALISED will not communicate via email, phone, or text after hours, on weekends, or during vacations.

Please reserve phone calls to the office for scheduling issues and short matters or you may be charged a fee of $20 per 15 minutes.
Coaching and Consulting Clients:
AXTUALISED is performing professional services.
NOT MEDICAL ADVICE: AXTUALISED is not treating or providing medical services, or advice. AXTUALISED is not acting as a medical provider (including doctor/physician, nurse, physician’s assistant, or any other health professional), mental health provider (including psychiatrist, psychologist, therapist, counselor, or social worker), registered dietitian or licensed nutritionist. Rather, we serve as a trainer, educator, coach, mentor and guide who provides life, health, wellness, business education and learning opportunities to our clients.
CONTACT A MEDICAL PROVIDER. You should consult a licensed health care professional before starting any health protocol or any health device such as those found on this Website. You are encouraged to confirm any information obtained from or through the Website and Services and review all information regarding any medical condition or treatment with your physician.
MEDICAL EMERGENCY. If you think you may have a medical emergency, call your doctor, go to the nearest hospital emergency department, or call the emergency services immediately- 911 or 988. If you choose to rely on any information provided by AXTUALISED you do so solely at your own risk.

Our liability to you or any third parties under any circumstance is limited to the lesser of one hundred dollars ($100) or the amount you have paid us in the twelve (12) months before the event giving rise to your to your claims.
